I’ve always wondered if there might be a genetic component to geographic tongue. My dad used to suffer with it too…
It’s one of the most common questions I see people ask about geographic tongue: does this run in families, or is it just bad luck? I went looking for what the actual research says, rather than the usual internet folklore, and the honest answer is: it’s complicated, but there’s real evidence for a genetic component.
Does Geographic Tongue Run in Families?
Yes, at least to some extent. Most people with geographic tongue report a family history of it, and studies back that up. One frequently cited study, from the University of Minnesota, found that first-degree relatives (parents, siblings, children) of people with geographic tongue had the condition at a rate of 14.1%, compared with just 4% in a control group — a meaningful difference.
That doesn’t mean it’s guaranteed to pass down, though. It means having a close relative with geographic tongue raises your own odds compared to the general population, not that it’s destiny.
So Is There a “Geographic Tongue Gene”?
Not one single gene, no — and this is where it gets more interesting than a simple yes/no answer.
Geographic tongue doesn’t follow a neat, predictable inheritance pattern like eye colour does. Researchers instead describe it as polygenic — influenced by multiple genes working together, more like height or skin tone than a single dominant or recessive trait.
There is one notable exception. A case study following one family across three generations found a specific mutation, in a gene called IL36RN, associated with geographic tongue in that family, inherited in an autosomal dominant pattern (meaning one copy of the altered gene was enough to be affected, though not everyone who carried it developed visible symptoms). It’s an interesting finding, but it’s a single family case study, not evidence that this mutation explains most cases of geographic tongue. Genetics research into this condition is still limited and early.
The Fissured Tongue Connection
If you also have a fissured tongue — deep grooves or cracks across the surface — that’s not a coincidence. Fissured tongue is the condition most commonly seen alongside geographic tongue, and researchers think the two may share some of the same underlying genetics. Older case reports have described families across three generations with both conditions appearing together, consistent with an inherited pattern.
Other Conditions Linked to Geographic Tongue
Genetic research has also turned up associations between geographic tongue and a handful of other conditions, most notably:
- Psoriasis — people with psoriasis have geographic tongue more often than the general population, and some studies have found a shared genetic marker (HLA-Cw6) in both groups, though this isn’t consistent across all research.
- Atopic conditions — things like eczema, asthma, and allergic rhinitis have also been associated with geographic tongue in some studies.
None of this means geographic tongue is a symptom of these conditions, or that having geographic tongue means you have or will develop them. It’s more that they may share some genetic ground.
Beyond the genetic research, this lines up with something I’ve written about separately: a lot of the classic geographic tongue triggers overlap with immune-related sensitivities. Genetics may set the underlying predisposition, and triggers seem to determine when and how often it actually flares.
Frequently Asked Questions
Can I pass geographic tongue on to my children? You might raise their odds, but it’s not a straightforward genetic handoff the way something like eye colour is. Many people with geographic tongue have children who never develop it, and plenty of people with no family history get it anyway.
Does it skip generations? It can appear that way, because the inheritance pattern isn’t fully understood and doesn’t follow a simple predictable rule. This is typical of polygenic conditions — they don’t show up in every generation just because the genetic predisposition is present.
If it’s genetic, does that mean nothing can be done about it? No. A genetic predisposition affects your likelihood of developing geographic tongue, not whether you can manage the symptoms once you have it. Is genetic testing available for geographic tongue? Not as a standard, useful test. The single-family gene finding mentioned above isn’t something GPs test for, and given how limited the current research is, genetic testing wouldn’t currently give you a clear answer either way. If you’re concerned, a conversation with your GP or dentist is more useful than seeking out a test.
The Bottom Line
Geographic tongue does seem to run in families, but not in a simple, predictable way — it looks more like a genetic predisposition shared across multiple genes, sometimes alongside fissured tongue, psoriasis, or atopic conditions, rather than a single inherited trait. Research in this area is still fairly limited, so treat any strong claims (including specific gene findings from single-family studies) with some caution.
